Check your blood pressure from the comfort of your home

NET Health’s high blood pressure self-monitor program offers a way for enrolled clients to check their blood pressure daily from the comfort on their own home. Clients are given access to a free blood pressure monitor and asked to collect and record their blood pressure readings daily. The readings are shared with their assigned Community Health Worker during their weekly phone chats. NET Health’s Community Health Workers provide enrolled clients with support, blood pressure education, navigation to resources, and guidance has clients adopt healthier lifestyle behaviors.
